I learned when I took my ACLS (Advanced Cardiac Life Support) course that one reason so many people with heart problems die on the toilet is because when you strain during a BM it slows your heart, sometimes too much.
Another is because people often retreat to the toilet when they feel bad, but not sure why. Sometimes a heart attack doesn't involve chest pain, but a feeling of being "unwell" and nausea.
